How Do I....
Register With The Practice?

Please bring your medical card and the receptionist can help you complete it. If you do not possess a medical card please ask at reception for an application form. If you would like to meet the doctor before registering, please make an appointment. Temporary residents are registered on a special form provided at reception. After registration you may see any doctor (regardless of with whom you are registered). All new patients are offered a check-up on joining the practice and patients will be given an appointment with the practice nurse. Please bring a small sample of urine in a suitable leak-proof container.

How Do I....
Change My Details?

If you change your address or surname, please notify the surgery as soon as you can so the records can be amended.

Please include your telephone number and postcode as it helps us to contact you if necessary.

Private Patients

Patients who do not wish to be registered as an NHS patient may request to be a private patient by liaison with an individual doctor.

Foreign Visitors

Foreign visitors may be treated as private patients. Please enquire about the current fee scale for this. Payment will be due at the time of service. Visitors from certain countries may be entitled to NHS treatment as temporary residents.
